12 month and 2-2½ Year and development reviews: Service update
We would like to apologise to families whose children have experienced delays in receiving their child's 12 month or 2-2½ year development reviews.
We know these reviews are an important opportunity to discuss your child's development and wellbeing, and we are working hard to ensure all affected children receive their review as soon as possible.
What happens next?
Families whose child's review has been delayed will receive a letter by post in the coming weeks containing:
- An Ages and Stages Questionnaire (ASQ), for you to complete, which helps us understand how your child is developing.
- An activity sheet with ideas to support your child's learning and development at home.
- A pre-paid envelope to return the completed questionnaire.
We are encouraging all families to complete the questionnaire. This will help us identify if you or your child needs any extra help or support.
Children who attend nursery
If your child attends a nursery or early years setting, we may contact them to gather additional information about your child's development.
If you do not want us to contact your child's nursery, please let us know when you return the questionnaire.
Returning your completed questionnaire
Please use the pre-paid envelope to return your questionnaire as soon as possible. If you lose your questionnaire or need support completing the form, please let us know. Our contact details can be found at the bottom of this page.
Once we receive your completed questionnaire, a member of the team will review the information provided.
Depending on the information received:
- If your child is developing as expected, we will update their health record and wait to see them at their next scheduled review.
- For those aged 12 months this will be at their 2-2.5-year review.
- For those older than 2.5 years, this is usually before they start school via an online questionnaire.
If we think your child may benefit from additional support, or if we need more information, a member of the team will contact you to discuss next steps. Where appropriate, we may offer an appointment with a Health Visitor.
I have concerns about my child's development
If you have concerns about your child's development, health or wellbeing, please contact the team. We are happy to discuss any worries you may have and can arrange support where needed at any time.

Frequently asked questions
Why has my child's review been delayed?
Some children did not receive invitations at the correct time following changes to our invitation process. We are sorry for the inconvenience this has caused and are working through all delayed reviews as quickly as possible.
Do I need to complete the questionnaire?
Completing the questionnaire is your choice. However, it helps us understand how your child is developing and identify any support they may need.
What if I do not return the questionnaire?
If we do not hear from you, we may contact you to check whether you still wish to take part in the review process.
I haven’t received a questionnaire or letter in the post
If you haven't received a letter by 1st October 2026, please contact us on the ‘one number’ 01905 520032 and choose the appointments team option who will be there to take your call 9am-1pm.
Will my child still receive support if they need it?
Yes. Any questionnaires indicating that a child may need additional support will be reviewed by the Health Visiting team, who will contact families to discuss the most appropriate next steps.
