Children in Care Service - Worcestershire

  • We support children and young people who are looked after by Worcestershire Children’s Services or placed in Worcestershire by another local authority.

  • Service hours: Monday to Friday 8:00am - 5:00pm

About our service

Our role is to support children and young people aged 0–18 who are looked after by Worcestershire Children’s Services, helping them understand and improve their health and wellbeing.

We carry out annual health assessments. These are full reviews that look at physical, emotional and mental health. Our aim is to help every young person thrive and reach their full potential.

During the assessment, we identify any unmet health needs and work with the young person and their carers to address them. This may include:

  • offering support and advice
  • connecting you to, or referring you to, other services

We can give advice on:

  • physical health
  • emotional wellbeing
  • relationships
  • sleep
  • healthy lifestyles
  • staying safe

After the assessment, you will receive a personalised health plan explaining how your needs will be met. We use each annual health assessment (or sooner if needed) to check progress and make sure the support is making a positive difference.

We also make sure health needs are heard and included in care plans. We support carers and professionals so everyone understands how to help the young person stay well. If there are extra worries or health needs, we act quickly to ensure the right help is in place.

Our team

We are a dedicated team of specialist community public health nurses. We support children and young people who are cared for by the local authority. We safeguard and promote the health and wellbeing of Worcestershire’s cared-for children.  

In England and Wales, the Children Act 1989 explains what a “child in care” means. People may also call them a “looked after child”. A local authority classes a child as a “looked after child” if it is responsible for their care or if it has arranged somewhere for them to stay for more than 24 hours.
